The CARE needs assessment is a 45-minute questionnaire covering living circumstances, employment, mental health, substance use, and more. The CARE staff utilizes motivational interviewing, trauma-informed practices, and restorative practices to work with the participant to identify their top three needs. Then, the CARE staff reaches out to appropriate service providers to address those needs and makes service referrals.
